However your request is sent, it must contain:
- The applicant’s postal and e-mail address and phone number,
- The name, address and SAS Mediation Solution membership no. of the professional you wish to mediate with;
- A brief statement of the facts. The consumer will tell the mediator what they want to achieve from mediation and why,
- Copy of the previous complaint,
- All documentsrelating to the request so it can be examined (purchase order, invoice, proof of payment, etc.)
